Dual antiviral therapy with pegylated interferon-2a and ribavirin only reaches sustained virological response rates of ~30% after liver transplantation. With the approval of viral NS3/4A (...) protease inhibitors telaprevir, boceprevir, and simeprevir and the NS5B polymerase inhibitor sofosbuvir, combination therapy offers new therapeutic options for HCV-infected patients, resulting in considerably higher sustained virological response rates in the nontransplant setting. Case presentation: We report three cases of TVR-based triple antiviral therapy in HCV genotype 1 reinfected patients after liver transplantation, of whom a 57-year-old Caucasian female and a 43-year-old Caucasian male were therapy naïve, and a 49-year-old Caucasian male patient was pretreated ineffectively. After 4 weeks of therapy, viral load decreased one to three log10 and became negative in weeks 6 to 8 in the therapy naïve patients. The pretreated patient showed a negative viral load in week 4. TVR was administered over 12 weeks, 750 mg thrice daily. Doses of immunosuppression with cyclosporine were reduced four to six fold. Initial peg-IFN and RBV doses ranged from 135–180 µg/week and 800–1,200 mg/day, according to the patient's body weight. Doses of peg-IFN and RBV were adapted to 90–135 µg/week and 400–800 mg/day after 2 to 12 weeks of protease inhibitor therapy. Dual therapy was continued for 36 weeks with total treatment duration of 48 weeks in the therapy naïve patients leading to a sustained virological response 12 weeks after the end of therapy. In the pretreated patient a breakthrough was detected in week 24 and therapy was discontinued. Overall, antiviral therapy was well tolerated. Side effects included dysgeusia and anemia leading to erythropoietin application and blood transfusions. Conclusion: This case series emphasizes that triple therapy with TVR is an efficient treatment for therapy naïve HCV genotype 1 reinfected patients after liver transplantation. These two books are among the most recently published tomes of a projected twenty comprising the first French edition of the Complete Works of Kierkegaard. Such a work represents the life-long dedication of Paul Tisseau, Kierkegaard's principal French translator. Many of Tisseau's translations have already been published in various other places, and it is generally known that he undertook to publish on his own several of the less commercially appealing religious works. After his death in 1964, his daughter completed his (...) work by correcting published and unpublished manuscripts and by translating the few works her father had not been able to do. The present edition was indeed worth waiting for; it is at once an artful translation and an excellent scholarly tool, complete with marginal pagination of the definitive Danish edition, the Drachmann, Heiberg, Lange of 1920-1926, plus location of each separate work in the first and third Danish editions. Jean Brun provides to-the-point introductions containing a wealth of historical material and ample footnotes referring to significant textual variations, subtleties of translation, and related passages in other works. Volume thirteen contains the Edifying Discourses in Various Spirits, Purity of Heart is to Will One Thing, The Lilies and the Birds, and The Gospel of Sufferings; volume eighteen, Two Discourses at the Communion on Fridays, The Woman that Was a Sinner, The Unchangeableness of God, For Self-Examination, and Judge for Yourselves.--C. Owen Barfield--philosopher, author, poet, and critic--was a founding member of the Inklings, the private Oxford society that included the leading literary figures C.S. Lewis, J.R.R. Tolkien, and Charles Williams. Lewis, who was greatly affected by Barfield during their long friendship, wrote of their many heated debates: "I think he changed me a good deal more than I him." Simon Blaxland-de Lange's biography (the first to be published on Owen Barfield) was written with the active cooperation of Barfield himself who, (...) before his death in 1997, gave numerous interviews to the author and shared many of his papers and manuscripts. The fruit of this collaboration is a book that penetrates deeply into the life and thought of one of the most important figures of the twentieth century. Recent reports published by the United Nations and the World Health Organization suggest that the brain drain of healthcare professionals from the developing to the developed world is decimating the provision of healthcare in poor countries. The migration of these key workers is driven by a combination of economic inequalities and the recruitment policies of governments in the rich world. This article assesses the impact of the healthcare brain drain and argues that wealthy countries have a moral obligation to reduce (...) the flow of healthcare workers from the developing to the developed world. Neuroscientists are searching for the engram within the conceptual framework established by John Locke's theory of mind. This framework was elaborated before the development of information theory, before the development of information processing machines and the science of computation, before the discovery that molecules carry hereditary information, before the discovery of the codon code and the molecular machinery for editing the messages written in this code and translating it into transcription factors that mark abstract features of organic structure such as (...) anterior and distal. The search for the engram needs to abandon Locke's conceptual framework and work within a framework informed by these developments. The engram is the medium by which information extracted from past experience is transmitted to the computations that inform future behavior. The information-conveying symbols in the engram are rapidly generated in the course of computations, which implies that they are molecules. (shrink)

Hawthorne and Magidor's criticisms of the model of presupposition and assertion that I have used and defended are all based on a rejection of some transparency or introspection of assumptions about speaker presupposition. This response to those criticisms aims first to clarify, and then to defend, the required transparency assumptions. It is argued, first, that if the assumptions are properly understood, some prima facie problems for them do not apply, second, that rejecting the assumptions has intuitively implausible consequences, and third, (...) that the 'margin of error' argument against the principle of positive introspection has a false premiss. The paper concludes with a response to a criticism of what Hawthorne and Magidor call 'the uniformity principle' that is used in the model to explain some pragmatic phenomena. (shrink)

Statisticians in medicine can disagree on appropriate methodology applicable to the design and analysis of clinical trials. So called Bayesians and frequentists both claim ethical superiority. This paper, by defining and then linking together various dichotomies, argues there is a place for both statistical camps. The choice between them depends on the phase of clinical trial, disease prevalence and severity, but supremely on the ethics underlying the particular trial. There is always a tension present between physicians primarily obligated to their (...) own patients (the weight of 'individual ethics') and ethical committees responsible for the scientific merit of the trial and its long-term implications ('collective ethics'). Individual ethics, it is proposed, favour the Bayesian approach; collective ethics, the frequentist. Though in some situations the choice appears clear-cut, there remain other where both methodologies can be appropriate. (shrink)

A comparison is made between the Bohm trajectory and Feynman path approaches to the long-standing problem of determining the average lime taken for a particle described by the Schrödinger wave function ψ to tunnel through a potential barrier. The former approach follows simply and uniquely from the basic postulates of Bohm's causal interpretation of quantum mechanics; the latter is intimately related to the most frequently cited approaches based on conventional interpretations. Emphasis is given to the fact that fundamentally different transmission (...) Ancillary care is care that research participants need that is not essential to make the research safe or scientifically valid and is not needed to remedy injuries that eventuate as a result of the research project itself. Ancillary care duties have recently been defended on the grounds of beneficence, entrustment, utility and consent. Justice has also been mentioned as a possible basis of ancillary care duties, but little attention has been paid to this approach. Abstract Physiological psychology has its conceptual roots in stimulus?response behaviourism. The resurgence of cognitive concepts in mainstream psychology has led to a separation between the two, largely due to the failure of most cognitive theories to specify how their explanatory processes could be realised in the nervous system. Connectionism looks as if it may be able to bridge this gap. The problem is that connectionism takes a radically different view of the brain from that adopted in traditional physiological psychology. Recent research on the relations of personality to well-being shows that the people who are most healthy, happy and fulfilled are those who are high in all three of the character traits of self-directedness, cooperativeness, and self-transcendence as measured by the Temperament and Character Inventory. In the past, the healthy personality has often been considered to require only high self-directedness and high cooperativeness. However, now the self-centred behaviour of people who are low in self-transcendence is degrading the conditions needed for (...) sustainable life by all human beings. Consequently, human beings need to and can develop their capacity for self-transcendence in order to maintain their individual and collective well-being. (shrink)
